How to Use a Penis Pump for ED: Step by Step, Including the Ring

Short answer: put the constriction ring on the open end of the cylinder first, add lube, press the cylinder against your body, pump slowly until you are erect, slide the ring off the cylinder onto the base of your penis, release the vacuum and take the cylinder off. The erection then lasts as long as the ring stays on, which should never be more than 30 minutes. That is the whole routine. Below we go through each step, with extra detail on the ring, because that is where most first attempts go wrong.

This guide is general information, not medical advice. If you have ED, a doctor can help find the cause; a pump can be used alongside most treatments.

What a penis pump does for ED

A penis pump, or vacuum erection device, is a clear cylinder with a hand or electric pump attached. When air is drawn out of the cylinder, the vacuum pulls blood into the penis and creates an erection. A tension ring at the base then keeps the blood in once the cylinder is removed.

It is one of the best-studied non-drug treatments for erectile dysfunction. Mayo Clinic notes that pumps can help most men get an erection firm enough for sex, with practice and correct use (Mayo Clinic). A 2025 meta-analysis of 18 studies found successful intercourse in about 80% of men whose ED had not responded well to tablets (systematic review, 2025).

What a pump does not do is make your penis permanently bigger. There is no evidence for that, and over-pumping to try can cause injury. We cover this in detail in the truth about "permanent gains".

What you need before you start

  • A pump with pressure control. Medical guidance recommends a vacuum limiter, so pressure can't get high enough to hurt you. An electric pump with a pressure display, like our automatic ED pump, also lets you see and repeat the level that works for you.
  • A sleeve that seals well. The soft opening at the base of the cylinder has to sit snugly around the penis. If it is too wide, air leaks; if the shape is flat, the scrotum can get pulled in.
  • Tension rings in more than one size. You won't know your size until you try. Most kits include two or three.
  • Water-based lubricant. For the seal and to help the ring slide.
  • Trimmed pubic hair at the base. Hair gets caught in the ring and breaks the seal.

Check with a doctor first if you take blood thinners such as warfarin or clopidogrel, or if you have sickle cell disease or another blood disorder. These raise the risk of bleeding or clots (Mayo Clinic).

Practise alone a few times before using it with a partner. The routine gets quicker and easier with each try.

  1. Load the ring onto the cylinder. Stretch the tension ring over the open end of the cylinder and let it sit just behind the rim. Do this before anything else: once you are erect, there is no time to fiddle with it. Some cylinders come with a plastic cone that makes this easier.
  2. Lubricate. Put a little water-based lube around the inside of the sleeve opening and at the base of your penis. This is what makes the seal airtight.
  3. Place the cylinder. Insert your penis and press the cylinder firmly against your body so the sleeve seals against the skin. Gently push the scrotum away from the opening with your fingers.
  4. Pump slowly. Start the pump and build pressure gradually. You don't need maximum pressure to get a full erection, and going fast is what causes pain and bruising. Many men pump for 30 to 60 seconds, release a little pressure, then pump again. It usually takes a few minutes.
  5. Move the ring onto your penis. When you are fully erect, keep the vacuum on and slide the ring off the cylinder onto the base of your penis, as close to your body as you can.
  6. Release the vacuum and remove the cylinder. Press the release valve or button so air goes back in, then slide the cylinder off. Never pull it off while the vacuum is still on.
  7. Start the clock. The ring can stay on for up to 30 minutes. Set a timer if you need to.

The ring step: where most first tries go wrong

The pump only creates the erection. The ring is what keeps it. If the erection fades as soon as the cylinder comes off, the ring is almost always the reason.

How to pick the right ring size

The ring should feel snug but not painful. Start with the middle size and adjust:

  • Too loose: the erection softens within a minute or two of removing the cylinder. Go one size smaller.
  • Too tight: pain, a burning feeling, or skin that turns dark purple quickly. Go one size bigger.
  • Right: the erection stays firm, the penis may feel slightly cooler, and there is no pain.

Our pump comes with three silicone tension rings (2.18, 2.64 and 3.17 cm inner diameter) for this reason. If you want to understand ring sizing in more depth, our cock ring size guide explains how to measure.

What to expect with the ring on

  • The erection can pivot at the base, because only the part above the ring is firm. Holding the base during penetration helps.
  • The penis may feel cooler and look slightly darker. That is normal.
  • Ejaculation can feel blocked or be uncomfortable, because the ring presses on the urethra. Rings with a small cutout on one side can help.
  • Take the ring off at once if you feel numbness, strong pain or coldness, and never sleep with it on.

Common mistakes to avoid

  • Pumping too hard or too fast. This is the main cause of bruising and small red dots under the skin (petechiae). Slow and steady works better.
  • The scrotum getting sucked in. Painful and very common with flat sleeves. Concave sleeves are designed to stop it; our guide on preventing scrotal suction has more fixes.
  • Forgetting to load the ring first. Trying to roll it on after the cylinder is off rarely works.
  • Not enough lube. No seal means no vacuum.
  • Keeping the ring on too long. 30 minutes is the limit, every time.

Side effects and when to stop

Pumps are safe for most men when used correctly. The most common side effects are small red dots, mild bruising, a cold or numb feeling with the ring on, and a sense of trapped semen (Mayo Clinic). In the 2025 meta-analysis above, bruising was reported by about 1 in 4 users, mostly mild. Stop and see a doctor if you get significant pain, large bruises, blood in your urine, or an erection that won't go down after the ring is removed.

Cleaning your pump after use

Separate the cylinder from the pump head, then wash the cylinder, sleeves and rings in warm water with a mild, unscented soap. Rinse well and let everything air-dry before storing. Keep water away from the pump head unless the manual says it is waterproof, and check the sleeves and rings for tears from time to time: a split ring or sleeve won't hold a seal.

Combining a pump with other options

  • With ED tablets: some men use both, and a recent review found that adding a pump to tablets improves results (Guillot-Tantay et al., 2026). Ask your doctor first.
  • With a penis sleeve: once the ring is on, you can slide a sleeve over the erection for extra shape and a firmer tip. We explain who this helps in penis sleeves for ED.
  • After prostate surgery: pumps are often used for penile rehabilitation, but the timing and routine should come from your surgeon.

Frequently asked questions

How long do you leave a penis pump on?

Only as long as it takes to get an erection, usually a few minutes. The cylinder comes off once the ring is on. The ring itself can stay on for up to 30 minutes.

How long do you stay hard after using a pump?

As long as the ring is on and fits well, so up to 30 minutes. Without a ring, the erection usually fades within a minute or two.

Do you put the ring on before or after pumping?

You load it onto the cylinder before pumping, then slide it onto your penis once you are erect, while the vacuum is still on.

Is a penis pump better than Viagra?

Neither is better for everyone. Tablets are more spontaneous; a pump works without medication, which matters if tablets don't work, cause side effects or aren't safe with your other medicines. Some men use both.

Manual or electric pump?

Both work. Electric pumps are easier if you have weak hands or want the same pressure every time; manual pumps give you a direct feel for the pressure. Whichever you choose, look for pressure control and a sleeve that seals well.

Can a penis pump make you bigger?

Only temporarily, during the erection. There is no evidence that pumps cause permanent growth.

Can I use a penis pump every day?

For sex, use it when you need it. Daily use without a ring is sometimes advised after prostate surgery for rehabilitation, but only on your doctor's advice.
